At SRQ Endodontics we make every effort to provide you with the finest care and the most convenient financial options. We accept most major PPO's and honor Careington 500 dental savings plan under specialist fee.
Please call our office to verify if you would be in or out of the network.
For in network patients, we will provide you with a pre-estimated based on the information you and your insurance have provided us. Your pre-estimated portion will be due at the time services are rendered along with any non-covered services and deductible if it applies.
If you are out of network, have any Medicare PPO or don't have dental insurance coverage, payment will be expected in full at time of service.
Please keep in mind that insurance is not a guarantee or method of payment, it is a contract between you and your elected insurance company. We will not enter into any dispute with your insurance, but we will gladly provide you with any paperwork needed to submit your claim. For PPOs except Medicare, we submit the paperwork as a courtesy to you but ultimately it is your responsibility.
CBCT Scans are usually not a covered service by most dental insurance therefor, if necessary for evaluation and or treatment it will be your patient responsibility and due at the time service is rendered.
We accept the following methods of payment:
American Express, Cash, Check , Care Credit, Discover, Master Card, Sunbit and Visa.
If you have any questions, please contact us prior to your appointment to verify your insurance and go over any questions or concerns. In regard to treatment fees, a formal treatment plan will be presented to you on your initial evaluation visit.
